Release from Liability - COVID

This release from liability (March 4 [the latest] copy is in the Download Files section of this homepage) is a pre-condition going forward before participating in in-person Kiwanis activities and future in-person Kiwanis meetings.  Any guests you may bring will also need to sign a separate waiver.  It only needs to be signed once and is good for the rest of the fiscal year.  It is modelled on the legal waiver sent to us from Kiwanis International.  You may print it, sign it and then mail it to John Barnes at PO Box 1266, Cheyenne, WY 82003-1266.  You may also email your scanned, signed copy to John Barnes at kiwaadmsect@cheyennekiwanis.org.
